I am in network with several insurance companies. Plans can be different for example, Aetna requires benefits to be verified before the visit. Others might have a co-pay or a deductible.

Aetna/Meritain - PPO, POS, OAP
SelectCigna - PPO, POS, OAP (benefits must be verified before the visit)
UHC/UMR - All Commercial plans throughout USA (many plan names including “Oxford, Empire, etc”)

Home visits available in NYC

$75 travel fee not covered by insurance

* Must be paid 24 hrs. before the visit

Self-pay

We do accept self-pay and will provide you with a superbill to submit to your Health Insurance Provider for possible reimbursement!

New home visits: $350

Home Visit Follow-up: $250

Telehealth: New Patient $200, Follow-Up: $175

Mini consults/Flange fitting: $75

*All self pay visits must be paid during scheduling
